Required Documents
In order to complete your application, copies of the following vital records are required at the time of registration:
- Online Application
- Application fee
- Birth certificate
- Students entering from another school must have copies of their elementary or secondary school record, recent standardized test results, and current grades. Parents should sign a Records Release Form authorizing Monsignor Clarke to request these records.
- Most recent physical
- Up-to-date immunization record
- Student Health History Form
- If the student is Catholic, a copy of the Baptismal certificate
- Incoming students in grades 6, 7 & 8 will be required to take a math placement exam.
