Required Documents

In order to complete your application, copies of the following vital records are required at the time of registration:

  • Online Application
  • Application fee
  • Birth certificate
  • Students entering from another school must have copies of their elementary or secondary school record, recent standardized test results, and current grades. Parents should sign a Records Release Form authorizing Monsignor Clarke to request these records.
  • Most recent physical
  • Up-to-date immunization record
  • Student Health History Form
  • If the student is Catholic, a copy of the Baptismal certificate
  • Incoming students in grades 6, 7 & 8 will be required to take a math placement exam.
